I am/My child is registering for 5 two-hour classes at a cost of $175. I understand that this price includes supplies and materials needed for learning techniques and completing 1 (one) project, and should be paid before the first class session.

I agree to release and hold harmless Kurrai Thompson and Kreated by Kurrai from any and all claims of liability related to any accident that may arise as a result of my or my child's participation in this class. I understand that I or my child will be using machines, needles, scissors, irons, and other tools related to sewing and that these activities include risks that can cause or lead to injury. I give permission for myself or my child to operate machines and to use tools and equipment. I also give permission for myself or my child to receive first aid and/or medical treatment if the need arises.

Photographic Release: I grant Kreated by Kurrai, its representatives and employees the right to take photographs of me or my child in connection with sewing classes. I authorize Kreated by Kurrai, it assigns and transferees to copyright, use, and publish the same in print and/or electronically. I agree that Kreated by Kurrai may use such photographs of my child with or without my child's name for an lawful purpose, including publicity, illustration, and advertising.

Refunds & Withdrawal Policy: You or your child may withdraw from this class up to 7 (seven) business days prior to your first class session. After that time, no refunds will be issued. All withdrawals and refund requests must be submitted in writing to Kreated by Kurrai.

Kreated by Kurrai reserves the right to suspend a participant without refund for inappropriate behavior. There are no refunds or credits for student absences.

Class Cancellation Policy: Kreated by Kurrai or the class participant may reschedule classes without penalty. Classes will be cancelled and rescheduled due to illness of the instructor or participant and in case of inclement weather. Notification via phone or email will be attempted if class is cancelled and/or needs to be rescheduled.
